As a Life Plan Community in CT, Elim Park offers everything from independent living to assisted living and full nursing care for seniors. Among the many services that distinguish Elim Park as a leader in senior living is our approach to the transition between total independence and the need for additional support.
When more care is needed, many Life Plan communities will encourage residents to move from their Independent Living apartment to a stand-alone assisted living building. At Elim Park, as personal needs increase, residents are able to remain in their independent apartment as long as possible through individualized services provided by our on-campus Assisted Living Services Agency (ALSA).
Residents can select as little or as many services as needed from our a-la-carte menu depending on their personal situation. Assisted living team members are employees of Elim Park and will help with activities of daily living such as medication reminders, showering assistance, escorts to shopping, and medical appointments. A qualified nurse meets with the resident to develop a customized care plan from a full menu of services that includes:

Assisted Living services: Assisted living provides some help with normal day-to-day activities such as medication management, housekeeping, bathing, dressing and personal care so that a person can still live independently. At Elim Park, we offer Assisted Living services in the comfort of a resident’s apartment, if they should ever need them.
Skilled Nursing (also known as Long-Term Care/Nursing Home): Residents in a skilled nursing center require round-the-clock care and monitoring. The skilled nursing center at Elim Park employs specialized staff (Nurses, LPNs and CNAs) to provide 24-hour care and supervision in a homelike environment.
Short-term Inpatient Rehabilitation: For individuals in need of intensive rehabilitation after a hospitalization, surgical procedure, injury or illness, we offer a 45- bed subacute rehabilitation wing on the Elim Park campus that is connected to many unique amenities such as dining options, an aquatic therapy pool and the Nelson Hall Performing Arts Center. All Assisted Living Services Agencies (ALSAs) in Connecticut are licensed and regulated by the Department of Public Health (DPH). They primarily serve adults age 55 and older who need some health or nursing care or assistance with activities of daily living, including dressing, eating, bathing, and transferring from a bed to a chair, but not the skilled care a nursing home provides. Only a DPH-licensed ALSA can provide assisted living services and ALSAs must operate within a managed residential community (MRC). Most Assisted Living facilities are stand-alone communities with a set monthly fee that includes room and board and a personalized care plan. However, Elim Park is unique as we bring customized Assisted Living services into residents’ current Independent Living apartments. Residents are able to enjoy the comforts of their own home while only paying for the Assisted Living services they need. Care plans are flexible and can be changed as needed.

This home care program is described by the state’s Health and Human Services department as a home care program to help people stay at home longer when the only other option would be to reside in a skilled nursing facility. Seniors 65 years and older who live at home or in an independent living facility who are CT residents would be assessed and the program would determine if they meet the criteria, both financially and with the amount of hands-on assistance they need with daily tasks by a home health care agency. This program is great to explore if a resident needs more assistance but has limited funds to pay for it. This program does not pay for room and board in a facility or the entrance fee. In addition to the carefree and maintenance free lifestyle that Elim Park offers, our Fitness & Wellness Center was designed to help residents achieve and maintain their optimal well-being. The Fitness & Wellness Center is open to residents, their families, and staff. We also offer community memberships for people 62 and over. Upon entering, the tranquil “water wall” welcomes and invites you to relax in our spa-like sitting area—a great place to unwind or read a book. You can have your hair done in the Center’s chic salon or enjoy the soothing hot tub. Even if you’ve never stepped foot inside a gym before, don’t worry! Our friendly and expert staff will welcome you and help support you in your fitness journey. We offer many levels of exercise classes, as well as aquatics classes in our heated pool all in a beautiful setting.
